To make the salted caramel cream cheese, place the cream, cream cheese, milk and salted caramel sauce in a measuring jug. Whisk (or use a hand frother) until thick but still pouring consistency.
Drizzle the inside of a tall serving glass with salted caramel sauce. Fill with ice and pour in the milk.
Pour over the chilled espresso and top with the salted caramel cream cheese. Garnish with pepper and sea salt flakes, to serve.
